City of Kingston Primary Care Clinic Expansion Grant

While not the responsibility of municipal government, the City of Kingston is committed to promoting the health, safety, and wellbeing of its residents by investing in initiatives that improve primary care access in our community. The Primary Care Clinic Expansion Grant funded by the municipality offers a one-time, financial grant of up to $100,000 to primary care clinics that demonstrate a comprehensive plan to expand patient rosters and enhance care accessibility through the use of technology, expanded administrative support capacity to address the burden of paperwork, and/or the addition of other allied professionals to the clinic.
The City of Kingston remains dedicated to ensuring accessible primary care for all residents and eagerly anticipates supporting local clinics as they expand their rosters and serve more patients.

Eligibility Criteria:
To qualify for the grant, primary care clinics must:
- Be an established primary care clinic within the City of Kingston that has been registered and operating for a minimum of 2 years using a practice enrollment model.
- Demonstrate a commitment to enhancing sustainable patient access to primary care in the community.
- Meet current Ministry of Health criteria for after-hours clinic access for rostered patients.
- Submit a comprehensive proposal including a detailed budget and a sustainability plan beyond the duration of the grant.
- Commit to roster an additional 1000 – 1200 patients over the course of the project year using Health Care Connect.

- Grant Amount per Project: Up to $100,000
- Application Period: May 7, 2024 - May 21, 2024
- Adjudication Period: May 22, 2024 - June 5, 2024
- Grant Announcement: June 12, 2024
Application Process:
- Complete the application form, providing detailed information about the clinic, its current services, and the proposed expansion plan.
- Include a completed budget form using the provided template.
- Submit the completed application along with any required supporting documentation by 11:59pm on Tuesday, May 21, 2024.
Selection Process:
Applications will undergo review by an adjudication committee and will be scored using the following criteria:
- Clear demonstration of the expansion plan.
- Feasibility and effectiveness of the proposed plan, including an estimated increase in patient capacity and a thorough budget review.
- Ability of the proposed plan to enhance primary care accessibility and patient care for unattached patients.
- Measurable outcomes such as increased patient numbers and initiatives that improve patient access to care.
- Sustainability of the project beyond the grant period.
